I have a pond 55' x 32' and 8" to 48" deep.
This is my pond's second year with fish. This
morning I found one of my sixteen Koi dead floating in the shallow end of my pond.
Looking over the fish, I did not see any signs of disease but did observe a small
hole located in the top of its head. The puncture size of the hole was that of
a small flathead screw driver or about a quarter of an inch.
Is it possible I received a visit from a Heron recently or is there something
wrong with my pond's environment?

Hi Ron, If
you live where we do, it's a little early for herons, but if your pond is below
the Mason-Dixon Line, predation is very likely. The fact
that there is only one fish dead and there's a definite sign of trauma reinforces
this. If you had a water quality or disease issue, you'd have multiple fish dead
